We are pleased to be able to offer Patients on line services.

These include:

Booking online appointments

Requesting online repeat medication

Viewing your online medical records including:

Demographics, 

Allergies and Adverse Reactions

Medication

Immunisations

Test results

Values - eg Blood pressure results

Problems/diagnoses

Procedural codes and codes entered in consultations.

Codes indicating referrals - please note that letters themselves are not shown.

PFS

Please note that with effect from 1st April 2016, the Practice will consider individual patient requests for access to detailed coded medical information via the online portal, Patient Access.

 

This is in addition to the existing services available via Patient Access.

 

If you would like to be able to view more than just your medication, allergies, allergic reactions and immunisations – which are already available to view via Patient Access – please make your written request to the Practice Manager/Deputy Practice Manager.

 

The decision to give access to detailed medical records will be made by your usual GP and you will be informed of the decision within 4 weeks of making your request.

 

In accordance with Wessex LMC guidelines and in order to ensure patient confidentiality for our young patients, Patient Access will no longer be available to patients aged between 11 and 16 years.

The Practice will NOT accept ANY Patient Access requests from, or on behalf of, patients in this age bracket UNLESS the request is made by the parent or guardian of an individual with a severe learning disability, in which case it is understood that parental responsibility should be maintained.
