Patients taking regular medications will require repeat prescriptions. For a safer and faster service repeat prescriptions are computerised. Please keep the right hand side of the form and when your medication is due send in the form with a tick against the items you require.
All requests must be in writing and may take up to 4 working days to process.
You can submit your request in the following ways:
- Bringing your request into the surgery
- Requesting it from your local pharmacy who can deliver it to us
- Faxing your request to 01329 854350
- Posting your request enclosing a SAE
- Requesting your prescription online via the link at the top of this page (if you would like to use the online facility, please ask at reception for a registration form).
Please note our receptionists are not allowed to take requests for prescriptions over the telephone .
REMEMBER – Your prescription is YOUR responsibility. Only order what you need. Unused medications cannot be recycled.
Electronic Prescription Service - Phase 2
We are now operating Phase 2 of the Electronic Prescription Service.
For many of you this will seem a similar process to the one you use now where the pharmacist collects the prescription from us saving you time and allowing you to collect directly from them.
The benefits of the electronic system is the speed at which the prescription can be sent, the risk of prescriptions going missing will be reduced and the pharmacists will eventually not have to collect from us. The prescriptions will be processed as they are now, but instead of printing a green form the doctors will sign it with an electronic signature on line.
Your pharmacist may ask if you wish to nominate them to receive your electronic prescription, they will record this information on the central spine which will update our records. They are also able to answer any questions you may have about the process. Pharmacies cannot nominate themselves to receive your prescriptions without your signed consent.
If you wish to use a different pharmacy, or take a prescription away with you we can issue a paper token, which can be given to a different pharmacy giving them permission to download the prescription for you to have your medication dispensed. You are free to change your nominated pharmacy at any time by signing a form at the new pharmacy.
We hope that disruption during this new process will be minimal and hope you will bear with us while we all get used to it.
Local Chemist Collection Service
Some of the local chemists operate a collection service. Please enquire at your local chemist. You will be asked to sign a consent form so that we know that you wish us to release your prescriptions to them. The chemists require 48 hours’ notice to process your request.
Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip. Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
Please allow four full working days for prescriptions to be processed and remember to take weekends and bank holidays into account. If there is a query with your prescription it may take longer to process.
