Appointments

Book an appointment via the Consulting Room

At Jubilee Surgery we offer a range of services with Doctors, Nurses and our Health Care Assistant.

Urgent same day Appointments

The way in which we now deal with our urgent same day appointments is through the Sovereign Health Network ‘hub’ which is based at Fareham Community Hospital. Please note that three practices in total operate through the ‘hub’ and therefore it could be a clinician from another practice who accesses your records.

The steps to achieve same day treatment, should you need it:

  • telephone the surgery on 01329 844220, should you require a same day urgent appointment
  • if you consent, you will then be placed onto the ‘hub’ telephone triage list, where a clinician will call you back. (please be aware that this is a same day service, please be patient as a call back may take a few hours dependant on the volume of the list)
  • await a call back, the ‘hub’ will only try to call twice and will come through as an unknown/private number.
  • you will then be triaged over the phone by a clinician who will give you advice, issue a prescription or make you an appointment.

**please be aware that if you miss both calls from the ‘hub’ then you will have to contact your surgery and be placed back on the list**

Routine Appointments

Routine appointments are released daily 4 weeks in advance at 10:00. If you have multiple problems you will need to book a longer appointment. These can be booked viacontact the surgery with non-urgent matters via the e-consult service.

Making evening and weekend appointments – giving you more choice

GP practices in this area are making it easier for you to make appointments at times which suit you.

From October, 2017 all practices in South East Hampshire, and Fareham and Gosport – including ours – will take part in a new scheme to allow you to make appointments from 8am to 10pm on week days, from 8am to 10:30pm on Saturdays and Sundays.

This extends the normal opening hours for surgeries, which until now have usually run until 6.30pm on weekday evenings.

If you do need to see someone, we will be able to offer you an appointment at your local ‘hub’ location – they are located at:

Fareham Community Hospital233A Brook Ln, Sarisbury Green, Southampton SO31 7DQContact Number: 01489 587433 Monday – Friday6.30pm to 10.30pm
Forton Medical Centre, GosportWhites Pl, Gosport PO12 3JPContact Number: 02392 5833333 Tuesday and Thursday – 18.30pm to 22.30pmSaturday and Sunday – 8am to 22.30pm
Portchester Health CentrePortchester Health Centre/West St, Fareham PO16 9TUContact Number: 02392 316069 Saturday and Sunday8am to 10.30pm

There, you will be seen by a locally-based healthcare professional – the service is led by GPs but also includes other staff such as nurses, or healthcare assistants. The staff can – with your consent – access your medical records, ensuring a high quality service.

Please note: the new extended opening hours is not a walk-in service – you need to make an appointment.

You can make an appointment by talking to our surgery staff and booking a slot in the normal way, or – if we are closed and you think you need to see someone more urgently – you can call NHS 111 to ask to make an appointment.
