In addition to the above times, there are a limited number of appointments available for GP’s and a variety of other healthcare staff on a Saturday morning, for those patients who do not find it easy to access the surgery during normal working hours. These clinics are being offered as part of Sovereign Health Network and are based at Highlands Practice; clinic times and the Highlands Practice address can be found below.

Making Evening and Weekend Appointments – Giving You More Choice

GP practices in this area are making it easier for you to make appointments at times which suit you.

From October 2017, all practices in South East Hampshire, and Fareham and Gosport – including ours – will take part in a new scheme to allow you to make appointments from 8am to 10pm on week days, from 8am to 10:30pm on Saturdays and Sundays.

This extends the normal opening hours for surgeries, which until now have usually run until 6.30pm on weekday evenings.

If you do need to see someone, we will be able to offer you an appointment at your local ‘hub’ location – they are located at:

Fareham Community Hospital, 233A Brook Ln, Sarisbury Green, Southampton SO31 7DQ

Contact Number: 01489 587433

Monday – Friday 6.30pm to 10.30pm
Forton Medical Centre, Gosport, Whites Pl, Gosport PO12 3JP

Contact Number: 02392 5833333

Tuesday and Thursday – 18.30pm to 22.30pm

Saturday and Sunday – 8am to 22.30pm

Portchester Health Centre, West St, Fareham PO16 9TU

Contact Number: 02392 316069

Saturday and Sunday8am to 10.30pm

There, you will be seen by a locally-based healthcare professional – the service is led by GPs but also includes other staff such as nurses, or healthcare assistants. The staff can – with your consent – access your medical records, ensuring a high quality service.

Please note: the new extended opening hours is not a walk-in service – you need to make an appointment.

You can make an appointment by talking to our surgery staff and booking a slot in the normal way, or – if we are closed and you think you need to see someone more urgently – you can call NHS 111 to ask to make an appointment.

Out of Hours Service

The responsibility for providing out-of-hours cover between 18:30 – 08:00 belongs to the NHS Out-of-Hours Service. You may call them direct on 111 or telephone the surgery where an answering service will give you instructions.

In a genuine emergency you should call 999. Chest pains and / or shortness of breath constitute a medical emergency.

Accidents and Emergency

If you are confronted by a serious problem such as severe chest pain or severe bleeding, call an ambulance (telephone 999) before calling the surgery and informing them.

Jubilee Surgery is not a walk-in clinic for minor injuries or illness.

If you arrive at the surgery in such circumstances you may be advised to attend the Accident & Emergency Unit at Gosport War Memorial Hospital.

Please note that any head injuries must go to Queen Alexandra Hospital.

Eye casualties must phone 02392 286000 (bleep 1027) before going to the hospital.
